New Patient Information

Your first appointment will consist of taking x-rays if needed and a thorough oral examination by the doctor. Since we have not seen you before, this comprehensive exam is designed to allow us to gather the information necessary to assess your individual need and provide safe, effective treatment. Please plan on being here for approximately 50 minutes. After this first examination we will schedule additional appointments as needed, including your first cleaning and/or recommended treatment.

Please plan to bring the following items with you:

Any current x-rays taken by your previous dentist, including bitewings taken within the last year and a panorex taken within the last five years. If you obtain previous x-rays, we ask that you have them sent directly to you, so you can be certain that they are available for your first appointment. If you do not have current x-rays with you, we will take them at this visit.

Complete insurance information including your insurance card so that we may take a copy for your chart.

Some form of payment, check or credit card, if you have a reduced fee plan or an insurance payment.

We do our best to give our patients with dental insurance a general idea of what would be covered with a typical insurance policy. However, given the large number of plans, we are unable to know what any individual policy includes or excludes. Therefore, it is important that you review your policy and have a good understanding of your benefits and limitations. Please remember that your policy is a contract between you and the insurance company; it is not between our office and the insurer. The responsibility for payment ultimately belongs to you.
